Student Travel Form
For any student travel outside of Boone that involves an overnight stay beyond Watauga County, the Office of the Dean of Students requires the collection of specific details for emergency contact purposes. Faculty or staff organizing field trips and other excursions must submit a participant list to the Office of the Dean of Students before the trip commences. This information is gathered for informational use only, allowing the University to identify participants during an emergency. Please note that submitting this list does not activate any form of insurance coverage.
